Swetha Pradeep, PharmD

Swetha Pradeep, PharmD (she/her) is a clinical pharmacist who provides comprehensive medication management at NACC. She collaborates closely with medical providers and patients to make sure patients are on the right medications for their health history. Swetha has a passion for serving a population with complex needs, interprofessionalism, and primary care. Her view on patient care is holistic, understanding that many external factors contribute to a patient’s health. She received her Doctor of Pharmacy from the University of Minnesota, and completed her residency training at Community University Health Care Center (CUHCC). Outside of work, Swetha likes to read, cook, bake, and hike with her dog.

At an appointment with a clinical pharmacist, Swetha reviews all of your medications and supplements to make sure they are appropriate for your health conditions, they are working as expected, that you are not having any side effects, that you are able to take the medication in the most effective way, and that you can afford your medication. Swetha can also assess potential interactions between prescription medications and traditional medicines.
