Our social services department at NACC consists of patient advocates that are here to assist with resources, community engagement, and insurance. Our patient advocates can help communicate and connect with workers on your community and medical team to help facilitate, advocate, and provide follow-up to ensure satisfactory quality of care. The patient advocates at NACC are also certified MNsure navigators and can help guide you through health insurance, applications, renewals, and additional questions. We strive to meet people with goals and support our patients to the best of our ability.
NACC patient advocates are available to help connect and make referrals for community resources. These referrals include:
Connecting patients and visitors with behavioral and chemical health providers
SSI referrals
Access to basic needs such as clothing and hygiene products
Exploring housing and shelter options through partnerships with other community agencies
Transportation support
Navigating county benefits
Addressing food insecurity
