Social services

Our social services department at NACC consists of patient advocates that are here to assist with resources, community engagement, and insurance. Our patient advocates can help communicate and connect with workers on your community and medical team to help facilitate, advocate, and provide follow-up to ensure satisfactory quality of care. The patient advocates at NACC are also certified MNsure navigators and can help guide you through health insurance, applications, renewals, and additional questions. We strive to meet people with goals and support our patients to the best of our ability.
