Behavioral Health

Notes

  • We have a new contractor supporting the BH department! Her name is Genevieve Lane— she can be reached at 5981.

  • BH appt zoom room is in BH clinic- please send patients to suite 103 in the Ancient Trader’s building if they need a zoom connection

  • BH registration is scheduling appointments - call BH front desk at 612-843-5981to see if any same days are available

  • BH has limited in person appointments, with rotating providers in each day. The majority of individual appointments and all groups continue to be on telehealth

  • Behavioral Health Home referrals (for patients who need long term behavioral health supports) Should be placed through orders. Please call Shawn at 612-843-6837 when making these referrals.

  • Tim and Todd have expanded availability for ADHD testing. Please place referral for NACC ADHD Testing in orders and ask patients to complete ADHD forms at our website: https://nacc-healthcare.org/forms

  • Diabetes management group- Thursday at 8:30- Alicia

Dental

  • Dental hours are 9:30-5 M-F

  • Same days and Walk ins available as the schedule allows.

  • Dental can help schedule pts when needed

  • Eastside clinic in St.Paul takes walk-ins 8:30-3 M-F for emergencies


Outreach

  • HIV Testing Event in Dollar Tree every Monday from 11 am -2 pm

      • Syringe exchange, HCV testing, COVID vaccines, & HEP A vaccines available as well as vouchers for further screening in clinic

      • Drop off winter gear (coats, boots, scarves, etc.) in DT during the next few Monday’s testing hours 11-2 for winter gear drive. Flyer available for accepted items.

  • Outreach at 5th & Lake encampment every Wednesday providing HIV & HCV testing along with other services with partnering orgs

      • Offering $25 Linkage to Care voucher to bring back to clinic to seek any type of service (See Nikki or Tina for gift cards)

  • Outreach at Avivo Village every other Thursday afternoon from 2pm-4pm

  • Street outreach every Thursday at Scooterville/Franklin Ave encampments

  • Outreach/ Linkage to Care support during The Aliveness Project’s Friday pop-up’s on 26th & 16th street from 10-1pm

    • Indigenous Health Toolkit Trainings for provider team from 8am-9am on the following dates

      • September 28th: Indigenous Health Equity, The Indigenous Patient-Provider Relationship focuses on the providers role in providing cultural safety and proposes models for clinical care using a more holistic, Indigenous focused lens

      • October 12th: Decolonization of Health Systems, addresses the need for change on a systemic level and imagines culturally safe and decolonized care environments, offering tools to advocate for patients on systems levels
