Jessica Horstmann (she/her) is a board-certified Family Nurse Practitioner. She graduated in 2016 from Yale School of Nursing with a concentration in diabetes management and completed her residency program at Community Health Center, Inc., in Middletown, Connecticut. She has worked as a primary care provider in Connecticut, Oahu, and Minnesota. She served as Director of Na Lei Kukui Nurse Practitioner Residency Program in Waianae, Hawaii prior to moving home to Minnesota in 2024. She has clinical interests in primary care for all ages, diabetes, women’s health, transgender care, chemical dependency, hepatitis and HIV. Her experience as an enrolled member of the Blackfeet/Piikani nation and growing up with a mother with quadriplegia has informed her stance on providing high quality care to Native and underserved people. She strongly believes in the importance of shared decision making and honoring the goals of her patients. In her free time, Jessica loves to bake, bike, and explore with her family.
