LICENSED THERAPIST

NACC is looking for a mental health professional with experience counseling individuals with significant trauma histories and possible co-occurring substance abuse issues.   We would also love to have someone with experience counseling children and their families.

NACC is an urban community clinic with a mission to reduce health disparities for Native American people in the Twin Cities.  Our mental health professionals are essential to this mission.  The counseling team is dynamic, with each member contributing and impacting the initiatives we take to improve the quality of care for our patients.

This position works in an integrated care setting, providing both outpatient mental health therapy in our counseling clinic, as well as behavioral health care in the medical exam rooms. The outpatient therapy role consists of conducting diagnostic assessments and providing individual, group, and family therapy to children, adolescents, and adults with complex mental health and substance abuse issues, and possible co-occurring chronic health conditions. Behavioral health care in the medical setting consists of providing brief screening and intervention on modifiable behaviors for both medical and mental health conditions. The counseling team also participates in the clinic’s population based care which entails interdisciplinary consultation as part of the care coordination for patients with complex chronic conditions.  We are looking for someone to work a minimum of 30 hours weekly and this comes with benefits.

The counseling clinic at NACC has a vacancy for a licensed therapist. The primary focuses of this position will be to provide mental health therapy in our outpatient clinic, in addition to providing integrated care in the medical clinic. Integrated care is brief screening and intervention on modifiable behaviors for both medical and mental health conditions in the medical clinic setting. This position will require experience working with young children.

Hours:  8:00AM to 6:00PM Monday through Friday, with the potential of one to two Saturdays a month from 10:00AM to 2:00PM

LICENSED DENTAL ASSISTANT

This position is a full-time, non-exempt position responsible for chairside assisting the dentists, dental students, dental therapy students and hygiene students.  This position reports to the Dental Clinic Manager.  This is a full-time position (80 hours per pay period), with health and dental benefits.  Hours are M-F from 8:30-5:00pm.

 

ATTENDANCE REQUIREMENTS

Attendance and punctuality are of the utmost importance.  Time off must be approved by the Dental Manager.

CERTIFIED MEDICAL ASSISTANT

The purpose of this position is to assist in the assessment, planning, implementation and evaluation of the delivery of patient care.   The CMA provides individualized patient care to promote the restoration of health and achievement of patient health goals. This position reports to the Direct Care Manager.  This is a full-time position (80 hours per pay period), with health and dental benefits.  Hours are 9:00am-5:30pm Monday through Friday, and rotating Saturdays after training period.

 PATIENT ADOVCATE

Primary Duties: This position is primarily responsible for assisting patients in addressing social issues including health insurance, transportation, and housing. This position reports to the PI Manager. This is a full-time position (80 hours per pay period), with health and dental benefits. Clinic hours are Monday thru Friday 9am-5pm and Saturdays 10am-2pm. Evenings and weekends are required occasionally.

ESSENTIAL FUNCTIONS

1.Responsible for acting as a social resource expert at NACC assisting providers and other staff with patient’s social needs

2.Plays an active role in care coordination as directed by Social Services Supervisor in finding resources and mitigating barriers for patients to comply with medical care recommendations.

3.Acts as part of outreach team promoting NACC in the community

4.Maintains current community resource information in an organized and accessible format (i.e. Insurance, pharmacy, traditional healing, counseling, housing, tribal registration, transportation, food assistance, etc.)

5.Meets with patients in the clinic to assess and assist with the patient’s social needs 6.Assists patients in completing medical insurance applications

7.Performs in reach and outreach to contact patients for social services

8.Assists patients with scheduling counseling, dental, and other support service appointments

9.Submits a monthly report of all encounters and activities to supervisor

10.Participate in performance improvement activities as assigned

11.Assist in writing reports for grant-related activity as assigned.

Experience: Preferred Bachelors in Social Services, Human services, Social Work or Related Field or one to two years related experience and/or training 2.Ability to read and interpret documents 3.Ability to speak effectively before groups of people 4.Ability to calculate figures and amounts 5.Proficient in Microsoft Office Not required, but a plus: 1.General knowledge of county programs (SNAP, GA, MFIP, etc.) 2.Case Management Experience 3. Clinical knowledge/medical terminology 4. Experience working with MNsure systems and/or Medical Assistance and MinnesotaCare Programs 5.Knowledge of community resources

OFFICE SUPERVISOR

JOB SUMMARY

NACC is searching for a warm and friendly office supervisor with excellent customer service skills. This individual will need the ability to maintain calm under stressful situations. This position is responsible for supervising the clinic’s daily registration functions, supervision of all registration staff and ensuring efficient Counseling clinic processes. The office supervisor should be self-directed and motivated around quality improvement and customer satisfaction, including facilitating access to care and billing for services.
